Instructions for Completing and Sending Patient Forms (Read Me First)
If you're on an iPhone and want to fill out the forms on mobile, please follow these instructions:
- Option 1:
- Click and download the form(s) you need to fill out.
- Once the form is open, click the bottom right three dots and select the "Share" button > View More
- Click the "Markup" feature. This will allow you to use a pen to check boxes, and click the top right corner's three dots and select the "Form Filling" option to fill out all of the blocks of text
- Once you're finished filling out the form, click the blue check box and "save file to" > Save the file to your phone and send it back to us
- Option 2:
- Click and download the form(s) you need to fill out.
- Once the form is open, click the bottom right three dots and select the "Share" button > View More > Save to Files
- Open your Files app on your iPhone and find the recently saved form & open it
- Use the pen or keyboard options at the bottom to fill in the blocks or check boxes as necessary
- Click the "Share" button and send it back to us
- Option 3
- Click and download the form(s) you need to fill out.
- Once the form is open, click the bottom right three dots and select the "Share" button > View More >
- Select another app to edit the form (such as Preview, Dropbox, or add it to a Google Drive and edit from there)
